How to register your product for the extended limited warranty: 1.

To receive your Extended Limited Warranty, purchase an in-stock Panasonic Lumix product named above between 4/1/2016 and 3/31/2017 from any authorized Panasonic Imaging Dealer (page 2) and take delivery on/by April 15, 2017. 2. Return this completed form (ALL FIELDS REQUIRED), including the original box label, and a copy of your sales receipt with purchase price circled. IMPORTANT: You must cut out and include the original box label from the product carton, which includes the UPC code, model number and serial number, or your claim cannot be processed. No copies will be accepted. 3. Only handwritten responses will be accepted. No sticker or preprinted address label. Product must be a new and unused purchase from an authorized Panasonic retailer listed above. 4. Product must be a “US Model” and carry a Panasonic USA Limited Warranty 5.
