FREQUENTLY ASKED QUESTIONS REGARDING RANK ONE ELECTRONIC FORMS
1. What is Rank One? Rank One is an internet based software employed by CCISD to keep track of student athlete eligibility, injuries, and inventory. This year, CCISD began utilizing Rank One electronic forms for students to submit athletic eligibility paperwork more conveniently. 2. How many forms do students have to submit? a. There are seven athletic eligibility forms for students to complete and submit. b. The medical history form should be completed online for 2014-2015 8th/10th/12th graders. c. For 7th/9th/11th graders, the physical/medical history form should be downloaded and printed from the “Download and Print” tab on the http://corpuschristiisd.rankonesport.com website. d. The other six forms should be completed online for all athletes regardless of grade. 3. How do students submit athletic eligibility forms? a. Students should navigate to http://corpuschristiisd.rankonesport.com with a parent. b. Students and parents should click on “Directions” and read them before beginning the forms. i. General Electronic Form Instructions: 1. In order to submit online paperwork appropriately, you will be asked to provide information such as home phone number, cell phone numbers, email address, name of primary physician, and other pertinent information. It is very important you have this information available during the time you plan on completing these forms. 2. Remember: It is important that you and your child fill out and sign all required spaces. Forms will not be accepted electronically if the forms are not completed correctly. 3. No student will be able to participate in an athletic period, practice, or games if these forms are not completed and processed. This is a University Interscholastic League rule. 4. To begin, click on the form name and fill out the information requested. It is important you have your student ID number available as it is required on each form. If you do not know the answer to a question, please answer the question with N/A or none. 5. To sign a document, click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over click on the refresh icon next to the signature box. If you are using a touch screen device, you may create an electronic signature, by holding your finger to the screen and signing your name or by using a stylus. 6. Once you have filled out all of the information on each page you will have the opportunity to print the document. 7. You will receive a confirmation email once the documents(s) have been reviewed by the Corpus Christi ISD athletic trainer for high school students or middle school coach for middle school students.

4. What happens after a student submits paperwork? a. After a student submits paperwork, their parent will immediately be sent a confirmation email from Rank One stating that each page has been submitted. b. After the middle school coach or athletic trainer views and approves/denies the form(s), their parent will receive another email from Rank One stating whether it was approved or denied. 5. What if a student does not know their student ID? a. If a student and/or parent does not know the student’s ID number, they can find their ID number on their approved class schedule or report card. 6. Who approves or denies paperwork? a. There are selected coaches at each middle school who approve and deny paperwork. Athletic Trainers handle approvals/denials at the high school level. 7. How does a coach approve or deny paperwork? a. A coach should access electronic forms on rankonesport.com b. Hover over athlete management and click on “Electronic Paperwork” c. Select your school and click “Show” d. Whether or not a form should be approved or denied is accessible via the “Rank One for Middle School Coaches” document provided to you. 8. Why am I receiving several emails from Rank One? a. Rank One automatically sends a confirmation email to the email address provided after each form is submitted. This confirmation email alerts you that the form was successfully submitted and provides the parent/guardian with a digital copy of the information for their records. b. After the form has been approved or denied by CCISD Athletics staff, the parent/guardian will receive another email for each form, notifying them of the form’s approved or denied status. Any forms which have been denied should be followed up upon via telephone by the athletic trainer or middle school coach reviewing the forms. 9. Who can I contact if I have questions? a.
