CHEF WAN A household name and with more than 25 years of experience in the industry, Redzuawan Ismail, or better known as Chef Wan is Asia’s most renowned culinary personality. He worked as an accountant before deciding to pursue a career in the food industry. He has an Associate Degree in Professional Chef Training and Hotel Management from the California Culinary Academy and a Ritz Escoffeir Diploma from the Ritz Hotel. After witnessing the popularity of Asian food in western countries, he decided to promote Malaysia and other South East Asian countries using food from their respective countries. He has appeared in various television cooking programmes, delighting audiences with his wit and humour while demonstrating his cooking skills. Due to his role as Malaysia’s representative at global gourmet events, World Expos and tourism exhibitions, Chef Wan became the first celebrity chef in Malaysia to be bestowed the title of Datuk by the Malaysian Head of State, Tuanku Mizan Zainal Abidin, in 2010. This was followed by his appointment as Culinary Ambassador by Tourism Malaysia later that year. Most recently, he was the recipient of Icons of Malaysia, a recognition given by the Malaysia Book of Records in 2011. Internationally, Chef Wan was presented with the Lifetime Jury Award at the 2007 Prix La Mazille for his contributions as Malaysia’s culinary ambassador. Section C [25 marks] [Time suggested : 50 minutes] Questions 26 -31 are based on the following passage (This passage is about a child who suffered from breathing problem and how he was saved.)

The 2011 holiday season was rough for Rece Mostek. The 19-month-old baby from Omaha, Nebraska, had spent much of November and December in and out of doctors' offices. Rece had been suffering from coughs, colds and fevers. His condition had worsened so much that he was hospitalised with pneumonia for four days in February 2012. Even when he was released from 5 the hospital, doctors warned his parents that he was not fully recovered yet. There was a chance that he might fall sick again. An hour after he arrived home, Rece began to cough non-stop and turn blue.

Jamie believed that his son was choking on a piece of Play-Doh that he was playing earlier. He then, called the hospital. While waiting for the 10 ambulance to arrive, he began to perform CPR. His quick action did not produce much positive effect. In fact, Rece’s condition began to get worse. When the ambulance arrived a few minutes later, the officers gave Rece oxygen and quickly rushed him to hospital. He arrived there in a critical condition. He was weak and did not have energy to move and he was still blue. 15

The doctor hurriedly gave treatment to Rece. She put a tube through the trachea to allow him to breathe better. Even though she had done that, Rece was still having difficulty breathing. Since the procedure made little difference, the emergency department doctor knew that there was a blockage that was preventing air from entering his lungs. However, she noticed that his oxygen 20 levels rose whenever she placed him on his right side. This indicated that the blockage probably was not in his throat. The doctors looked at the X-ray result and the chest X-rays revealed the problem was in Rece's lungs. His right lung was red, sore and hot from pneumonia and began to become worse every minute. From the X-ray too, she did not find any foreign object on the right lung 25 and she suspected that the left lung might be blocked.

The doctors had to move quickly. Rece was facing a great risk whereby 10

his brain would be damaged due to lack of oxygen. The risk of brain damage increased each time his oxygen levels began to fall. Since they could not do anything much, the doctors decided to send Rece to another children’s hospital. 30 A transport team rushed Rece to the paediatric intensive care unit at a children’s hospital about 20 minutes away. There, a group of specialists would try to perform surgery to remove the blockage. Once they arrived there, Rece was placed on extracorporeal membrane oxygenation (ECMO), a bypass pump that provides oxygen when a patient's lungs can no longer function. For the next 45 35 minutes, paediatric pulmonologist Dr Paul Sammut and a team of surgeons tried to hold and remove the object with a variety of medical tools. Dr. Sammut finally succeeded doing that with a tool used by urologists to remove kidney stones. 5

Meanwhile, the Mosteks spent a frantic hour trying to understand the terrible nature of Rece's situation. Then Dr. Sammut entered the waiting room 40 and gave the worried parents a thumbs-up. With him was a plastic bag and inside it was the unbelievable item that had nearly taken Rece's life: a popcorn kernel that had been stuck in his left lung.

Rece's condition was the result of a perfect storm. A few months earlier, his parents gave him popcorn. Unfortunately, he had inhaled a kernel of 45 popcorn, which later was stuck in his right lung. This led to infections and breathing problems, including pneumonia. But his excessive coughing earlier that day had pushed the kernel from the right lung and propelled it into his left lung, where it unluckily blocked his breathing airway.

Today, Rece's breathing is strong, and there is no sign of brain damage 50 from oxygen deficiency. A tiny scar on the side of his neck where the breathing tube was inserted is the only trace of his ordeal. The Mosteks are much more wary of giving him popcorn now. "Whenever I see people feeding their young kids popcorn and not paying attention, I want to warn them to be careful!" says Rece's mother, Brenda. 55
