Ventura Street — Ojai, CA Members of the public wishing to address the City Council/Successor Agency Board on items appearing on the agenda are requested to complete a "speaker's card" and file it with the City Clerk prior to the start of the meeting. Options to Provide Short-term and Long-term Disability Coverage to FullTime City Employees (continued from October 13) RECOMMENDATION: Authorize the City Manager to obtain short-term and long-term disability insurance for employees via the City's insurance broker, Shanbrom & Associates, through Lincoln, at the quoted rate of $0.29 per $10 benefit for short term, and $0.23 per $10 weekly benefit for short term.

Cancellation of Upcoming Regular City Council Meetings (continued from October 13) RECOMMENDATION: Discuss and determine the schedule for future City Council meetings, and adopt a resolution determining the regular meeting dates to be cancelled.

Discussion of Law Enforcement Activity Related to Shangri-La Care Cooperative and Consideration of Sending a Formal Request for Release of Information by the Ventura County Sheriff's Office RECOMMENDATION: Consider adoption of a Resolution authorizing the Mayor to send a formal request for Release of Information by the Ventura County Sheriff's Office related to Shangri-La Care.

Ojai Redevelopment Successor Agency (RDSA) On January 10, 2012, the Ojai Redevelopment Successor Agency was deemed as the former Ojai Redevelopment Agency by Resolution No. 12-03, under Health and Safety Code section 34173(a). The same rules as for City Council meetings apply.
