1.0 INTRODUCTION 1.1 Project Overview Sublette County Rural Health Care District (District) intends to procure a qualified Design Builder (DB) to provide complete design and construction services for a hospital The fixed-limit, not-toexceed design-build budget for construction is $17,000,000.00 (seventeen million dollars). Total project cost including soft costs, medical equipment and furnishing is estimated at $28,000,000 (twenty-eight million dollars). 1.2 Key Organization Personnel The District currently operates the Pinedale and Marbleton/Big Piney Medical Clinics serving primarily Sublette County, Wyoming. These are free standing physician clinics with 24/7 emergent care (ER) and ambulance services. The buildings and grounds are owned by the County of Sublette and rented to the District. District Board Chair – Laura Clark Building Committee Chair – Scott Scherbel Client representative - TBD The successful DB will contract directly with the District. 1.3 Proposed Project Schedule The preferred project schedule is to begin construction by TBD and complete construction by TBD. The final project schedule will be developed by the Client in consultation with the selected DB. 1.4 Construction Overview 1.4.1 The exact square footage of the hospital has yet to be finalized. However, the departments/spaces listed in Appendix A are the preliminary operational and spatial needs of the facilities to meet the DISTRICT’S goals. 2.0 INSTRUCTIONS TO DBS 2.1 Introduction  The project will take place at a yet-to-be determined location in Pinedale, Wyoming. 

The Fixed Limit of Construction Cost (FLCC) for the CM/GC is $17, 000,000 (Seventeen million dollars).

Contact with individuals other than listed may result in Proposal disqualification. Addenda, as DISTRICT deems necessary, will be issued to the registered DBs via email. Such addenda shall be incorporated into and form part of this RFP. The DISTRICT shall not be held responsible for any oral instructions. DISTRICT reserves the right to modify or change the content of this document prior to the date the proposals are due; to reject any or all proposals, to waive any informality or irregularity in any proposals received, and to be the sole judge of the merits of the respective proposals received.

SCRHCD RFP - Design Build Email proposals are required. Hard copies may also be submitted as supplemental material but do not need to meet the deadline. Email proposals received after the closing time will be considered disqualified and returned unopened to the DBs. The time and effort to prepare and present the Proposal is solely the responsibility of the DB. Under no circumstances shall DISTRICT be obliged to reimburse such expense. 2.4.2 Minimum Qualifications Where required by state law, DBs and key team members must be licensed to work in the State of Wyoming. DBs shall possess an understanding of and have current building experience with local, state and federal healthcare building codes. They must also have experience working on similar projects in the State and be familiar with the rules and regulations of the Wyoming Department of Health for the construction of hospitals. Experience with third party reviewers and inspectors in lieu of Wyoming Dept. of Health reviews and inspections will be beneficial. Experience with USDA financed projects is also beneficial. Team members shall be experienced in the construction of acute care and critical access hospitals with construction costs of $17 million dollars. Relevant experience must have occurred within the last three (3) years. 2.4.2 Form of Proposal Proposals are not to exceed 30, 8.5”x11”, pages in length and are to address the following information. Full resumes for key members may be submitted as an appendix. Covers and tabs (with no text) do not count within the 30-page limit. 2.4.2.1 Firm overview, background and values 2.4.2.2 A description of key members, their roles and a brief description of related experience. These key members include contractor, architect, mechanical and electrical engineers, interior designer, construction cost advisor, construction project manager and construction superintendent. Include a team organization chart and description of the working relationship of the team members. 2.4.2.3 A description of four (4) current/recent, directly related health care projects from FIRM , identifying the philosophies, construction costs, completion dates and participation of team members proposed on this project. If proposal includes a joint venture where design and construction are separate firms joined together for this project, special emphasis will be placed on projects where the Designer and Contractor worked together. Include a contact name, number and email address of the Owner from each of the four projects. 2.4.2.4 A statement illustrating your understanding of the project and the goals of the Client team. 2.4.2.5. A statement of your proposed design/build approach. SCRHCD RFP Design Build

SCRHCD RFP - Design Build 2.4.2.6. A statement of your proposed project management methodology including cost, schedule and quality control. 2.4.2.7 Describe your close-out and warranty procedures. The DISTRICT desires that the projects described in the RFP building have a warranty for a period of two years commencing from the time of acceptance by the DISTRICT. Note: Construction as-built must be stamped by a licensed professional engineer in the State of Wyoming certifying quality assurance and code compliance. 2.4.2.8 A statement listing the number and date of each of Addenda received. 2.4.2.9 A statement as to the DB’s litigation background over the previous five (5) years, and the DB’s bonding and insurance coverage (letters/certificates may be submitted in an appendix). The DB Contractor must certify that there has never been a claim against their bond. The DB must also certify that neither the firm nor the principals of the firm are presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from soliciting work by any governmental department or agency. The firm must also certify that neither it nor its principals have been terminated during the performance of a contract, or withdrew from a contract to avoid termination. In the case of a joint venture proposal, all included firms must provide this certification. If firm cannot certify these three statements, the firm shall submit a written explanation of the circumstances for review by the DISTRICT. DB will be required to execute an agreement or agreements to indemnify and hold the DISTRICT and the Sublette County harmless against all claims arising out of the design and construction of the facility until acceptance of the completed project, including, but not limited to, claims from DB's employees, contractors, sub-contractors and thirdparties. 2.4.2.10 Please propose the following fees for the DB team for this project as a percentage of the fixed-limit, not-to-exceed design-build budget for construction of $17,000,000: Architectural / Engineering ___________% Construction/ Pre-Construction ________% Construction Profit/Overhead _________% These fees will be fixed and depicted as a line item in the budget estimates and provided by the DB throughout the design, estimating and construction process.

SCRHCD RFP - Design Build 2.5 Evaluation Criteria The proposals and interviews will be evaluated on the following basis. The same criteria will be applied in assessing the proposal and the interview. DB scores after proposal submission will be used to shortlist DBs for interviews; DB scores after interviews will assess both the proposal and the interview and will be used to select the successful DB. The criteria below will be used to evaluate each DB’s proposal; yet will not be the sole determining factor in the selection of the DB. 2.6 Interviews Interviews will be held December 15, 2017. DISTRICT will schedule the interviews. Each interview will be a total of approximately 1.5 hours in duration roughly composed of equal parts of presentation and questions and answers. Please bring key team members, as noted above in 2.4.2.2, to the interview and prepare a presentation to address your understanding of and approach to the project.

3.0 SCOPE OF SERVICES 3.1 Basic Services 3.1.1 Design-Build management services, which includes overall team management and compliance, estimating, value analysis services including but not limited to, value engineering, cost savings suggestions and best value recommendations, constructability studies, construction risk mitigation, scheduling, interdisciplinary coordination, review and compliance with all applicable codes and standards including but not limited to, the Wyoming Department of Health, State Fire Marshall, Municipal, USDA and CMS as well as coordination of the contract documents, subcontractor management, inspection and certification and warranty implementation. Funding is expected to be provided by the U.S. Department of Agriculture Rural Development and its requirements must also be met. 3.1.2 Architectural, Interior, Structural, Mechanical and Electrical, Civil and Landscaping Design. SCRHCD RFP - Design Build 3.1.3 Complete design and construction to have the facility open for operation on the pre-determined project completion date of ______________ (TBD). 3.1.4 Appendix A – entitled “Scope of Services” defines the services to be included in the Base Fee in further detail. 4.0 SPECIALTY CONSULTANTS Specialty consultants, such as medical equipment design/installation services, furnishings design/installation services, etc., may be engaged separately by the DB as needed. As part of the contract, the DB will work closely with the DISTRICT to coordinate the construction of the structure(s) needed for the separate installation(s) of the equipment and/or furnishings. 5.0 CONSTRUCTION COST ESTIMATING Construction cost estimating will be provided by the Design/Builder to align current estimates with the Client’s budget at each project milestone. 6.0 FEE STRUCTURE The Client’s intent is to compensate the DB in accordance with generally accepted practices. The DISTRICT has requested a proposal for a Fixed Fee for Construction with a percentage of the fixed Construction fees for the Architect and Engineering team, the Pre-Construction Services from the Contractor and the Profit/Overhead of the Contractor. 7.0 BONDING REQUIREMENTS Bid bond security, in the amount of five percent (5%) of the Fixed Limit of Construction Cost, made payable to the Sublette County Rural Healthcare District. THE BID BOND MUST BE ON THE BID BOND FORM PROVIDED IN THE PROCUREMENT DOCUMENTS TO BE CONSIDERED AN ACCEPTABLE PROPOSAL. If the bid bond security is submitted on a form other than the Owner’s required bid bond form, and the bid security meets all other legal requirements, the contractor will be allowed to provide an acceptable bid bond by the close of business on the next business day following notification by Owner of submission of a defective bid bond security. 8.0 WYOMING RESIDENT LABOR PREFERENCE The DB must certify that they are familiar with the Wyoming Resident Labor Preference Laws and will abide by the current statute throughout the time line of the project. WYOMING STATUTES: SECTION 16-6-201 (2013); This act may be cited as the “Wyoming Preference Act of 1971”

SCRHCD RFP - Design Build 9.0 APPENDIX “A” CONSTRUCTION COST ESTIMATES SECTION A – BUILDING CONSTRUCTION COSTS –the DB will develop refined construction cost estimates as follows: A. CONSTRUCTION ESTIMATE Estimate will include building construction for hospital, general conditions costs, contractor’s insurance, contractor fees (pre-construction & construction management), 3.5% estimating contingency, 3% construction contingency and building permit/plan review fees. SECTION B – SITEWORK, PARKING, LANDSCAPING B. SITEWORK, LANDSCAPING Estimate will include site demolition, temporary fencing and barricades, traffic control, earthwork, asphalt paving, curb and gutters, sidewalks, site furnishings, landscaping/irrigation, and signage plus general conditions costs, insurance, and contractor fees. SECTION C - IT / TELECOMMUNICATIONS – BY DB C. ESTIMATE OF IT / TELECOMMUNICATIONS INFRASTRUCTURE AND HARDWARE C.1- Estimate will include all necessary infrastructure and hardware in support of items including but not limited to phone systems, CATV cabling, computer network cabling, nurse call system and security system integration. C.2- Equipment, such as computer workstations, data network switches, servers and other computer hardware and software. SECTION D - SURVEY, TESTING AND INSPECTION FEES – BY DB D. ESTIMATE OF SURVEY, TESTING AND INSPECTION FEES – BY DB D.1- Survey (verify) grade/floor elevations, utility locations, building corners/edges). D.2 - Geotechnical Investigation Report – update and verify foundation conditions. D.3 - Construction Materials Testing (concrete strength, soils compaction, steel/masonry testing). D.4 - 3rd Party Design review and Building Inspections. D.5 - Independent HVAC/Plumbing Commissioning/Test & Balance.

MEDICAL EQUIPMENT Estimate will include new major medical equipment (fixed & moveable) and minor equipment plus re-use of existing equipment and includes professional fees. SECTION F - FURNISHINGS F. FURNISHINGS Estimate will include interior moveable furniture and artwork. SECTION G - PROFESSIONAL FEES & EXPENSES G. PROFESSIONAL FEES & EXPENSES G.1 – Fees will include Architectural, Engineering and Consultant fees for Architectural, Structural, Electrical, Mechanical Design, Civil and Landscape Design. G.2 – Fees will include Furnishing Design Services (furniture selection & spec data for Owner purchase of furnishings). G.3 –Fees will include Medical equipment planning (equipment selection, utility coordination & spec data for owner purchase). G.4 – Estimate will include a not-to-exceed fee for Reimbursable Expenses for design team.
