SHELBYVILLE HIGH SCHOOL SUMMER SCHOOL 2017 – ENROLLMENT FORM Name ________________________________________________________________________ Last First Middle Home Phone # _________________ Emergency Phone # _________________ Shelbyville High School is offering the opportunity for 8th grade students to take one credit of Physical Education (PE), Health & Wellness, or Digital Applications & Responsibility (DAR) during summer school in order to free up space in their four-year plan. Enrollment will be on a first-come, first-served basis and is limited (50 students each for Health and PE and 28 maximum for DAR). If you enroll and do not complete a course in summer school, it will be scheduled during the sophomore year. No schedule changes will be made at the beginning of freshman year if you fail to complete the course you signed up for during summer school. Students who enroll in PE or DAR are expected to attend class daily for a 2-hour morning session. Completion of the Health & Wellness course will be done through the online APEX program. Interested students should be self-motivated and willing to commit at least 40 hours outside of class to complete the course during the summer school session. Students will not be expected to attend class daily; however, students will be required to attend the first three days of summer school for special training sessions as well as take their tests and quizzes at SHS. Classes begin on Thursday, May 25, 2017 and end on Thursday, June 22, 2017. This form and book rental fees will be due on Thursday, March 23, 2017. Checks should be made payable to Shelbyville High School. Mrs. Ragin will collect all fees in the SMS front office.
